(Directions: Complete all portions of the form. Please specify certified or uncertified. For an uncertified excerpt, the clock start/end timeframe must be provided in the request. Please note that uncertified transcripts cannot be quoted in court filings/papers. Excerpts are available only for uncertified, unquotable transcript orders. An invoice estimate will be sent for you to process payment via credit card or by check. Once payment is received and on deposit, transcription will begin. This form may also be used to receive an estimate for and pre-order reporter services for an upcoming hearing/trial -- daily/realtime, etc. -- for which a deposit can be provided.)
